The RAR file contains ISOs of the four installation discs needed to play The Longest Journey. 1- Open and extract with WinRAR (or alike program). 2- Use Daemon Tools (or Alcohol, or an alike program) and mount TLJ_US_1.mdf. (Alternatively, you could burn a discs for the ISO.) 3- Run Setup.exe from TLJ_US_1.mdf. 4- Upon prompt for next disc, mount each MDF file in chronological order. 5- Finally, replace the default Game.exe file with the patched Game.exe.
